Joined up health, care and housing services at local level
Our vision is the strategic integration of the components which meet society's aim of keeping people living longer healthier and independent lives at home. A single source of support that brings together healthcare, personal care, telecare, aids and adaptations will maximise benefit, reduce time in hospital and make better use of limited resources.
